*Step by Step Process on How to complete Your Student Attendance
Please press firmly because the attendance sheet is in triplicate form;
- Write your case workers name at the top left hand corner of the sheet;
- Check the box of the office which you are located in;
- Fill in your name (first and last);
- Fill in your Social security Number (CalWORKs office will not use this number but will only use your ID #);
- Fill in the activity location ( this should always be Porterville College);
- Report Period From should be a two week span (please refer to the example);
- Under the days of the week, please indicate what day of the month corresponds with the day of the week;
- Please fill in your class schedule for each day- Note: if your class starts at 8:20am please put down that it starts at 8:30am;
- When filling in your class schedule it should always be rounded off to the nearest half hour;
- When there is more than an hour difference between classes please separate them on the attendance sheet; example is Wednesday, March 5;
- Add up daily total hours vertically and add the total hours horizontally at the end;
- In the “comments” section please note any work study on campus, homework hours in the library, and clinical hours; please attach sign in and sign out forms with the attendance sheets;
- Sign your name and date the attendance sheet the day you turn it in to the CalWORKs office;
- The activity supervisor’s signature will be a CalWORKs counselor;
- Please turn in on or before the attendance due date which is the last day of the month-
- Refer to Student attendance due dates;
- A counselor will then take it to the TulareWorks Office. You can also pick up your signed attendance sheets and take them to your perspective office but please allow 2-3 business days to allow the counselor’s time to review the attendance and sign them.

**Steps to Complete Your Travel Log
- Write your case workers name on the top right hand corner of the form;
- Check the box that indicates which office you are located in;
- Write your name;
- Enter your social security number;
- Write your address (this should be your home address);
- Fill in your case number;
- Write the date ;
- For destination and purpose please indicate your starting point, and ending point for that day (if you work off campus the CalWORKs counselor’s cannot sign your travel and this mileage will have to be signed by your employment supervisor);
- When turning in your travel log, you can only submit child care and school mileage to the CalWORKs counselor;
- Write down your odometer reading, you should probably take the reading when you start off in the morning and when you end at night;
- Add up your total miles.
