Jennifer Franco, Hospice Liaison
Jennifer is the initial contact with any potential patient or family that is considering
comfort focused care for their loved ones. She is also responsible for building relationships
within the community, educating physicians and families, and discussing hospice philosophy
and comfort versus curative measures relevant to hospice care.
Richard Freitas, LCSW
Richard provides support and resources for patients that come onto hospice services,
including assisting with placement, family dynamics, community based resources, and
assisting the family and understanding and coping with the end of life process.
Vicente Pacheco, RN Cinical supervisor
Vicente is part of our leadership team as a clinical supervisor. He assist in managing
the various clinical disciplines, assisting with appropriateness for services, change
of condition and patience, overall education to our clinical staff, and much much
more. Vicente will be sharing some of the information regarding overall decline of
condition, clinical criteria for coming onto hospice services, and address some of
the misconceptions associated with hospice in general.
Thomas Olson, Spiritual Advisor
Tom is one of several of our pastors that is available for spiritual needs and support.
Not only does he provide end of life, comfort and support, but also makes regular
visits to patients to address any spiritual needs, provide comfort, and support families
and loved ones as they transitioned with the patient through the next stages of their
life.
Description
A Panel from Bristol Hospice will discuss not only the physical components of end-of-life
care, but also the emotional and psychological aspects of not only the patient, but
also their support system and loved ones.
